Appointments
Please call our office at (480) 947-7725 to schedule an appointment. Every effort will be made to acccomodate your schedule. If it is necessary to reschedule or cancel an appointment, we ask that you give us a 24 hour notice.

Insurance and Billing
We accept Medicare assignment and most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own dermatologist.

Medicare and most commericial insurance accepted. Please contact us about specific insurance plans. Please bring your insurance card with you to your appointment.
Payment Options
Payment of all copays, deductibles and coinsurance is expected at the time of service. We accept cash, checks, Visa and MasterCard.
